Summer Clerkship of UPJS MF students in the Academic Year 2014/2015

1. Obligatory Summer Clerkship of UPJS MF students is to be taken in the period of summer holiday. The tudents wishing to take it abroad are requested to get approval from a subject´s coordinator on „Request for permission to perform Summer Practice abroad“ in advance and submit it to the Study Department.

2. Students of General Medicine and Dental Medicine are requested to apply for Summer Clerkship at a chosen department personally. „Request to take Summer Clerkship in the Academic Year 2014/2015/Request for permission to perform Summer Practice abroad“ are to be given to the Study Department till 15.04.2015. In case the student wishes to take Summer Clerkship in Slovakia – it is possible to take it in the University Hospital of L. Pasteur in Košice and in the hospitals contracted with UPJS MF in the Ac. Year 2013/2014. If Summer Clerkship is to be taken in Slovakia, however not in in the University Hospital of L. Pasteur in Košice are requested to print twice „Summer Clerkship Contract (Zmluva o odbornej praxi)“ from the web: Študenti – Organizácia odbornej praxe v AR 2014/2015 http://www.upjs.sk/lekarska-fakulta/informacie-pre-studentov/odborna-prax , fill it and submit approved ones by the respective hospitals to the Study Department till 15.04.2015.
3. Length of Clerkship is specified in the study plans and must be complied with the study plans.
4. Clerkship dates approved by the respective departments/hospitals must be kept.
5. In case of Summer Clerkship taken in Slovakia, however not in in the University Hospital of L. Pasteur in Košice will get the contract signed by the Dean by 31.05.2015 at the latest.
6. Summer Clerkship must be signed in „Záznamník klinickej praxe“. In case you miss it – possible to collect it at the Study Department.
7. „Evaluation of Summer Clerkship“ along with „Záznamník klinickej praxe“ are to be submitted to the subject coordinators till 21.08.2015 at the latest. The subject coordinators will register the final evaluation into „Záznamník klinickej praxe“ and into AIS.
8. In case the Slovak hospital/department where student would perform summer clerkship would request a payment for clerkship – it is necessary to inform a study referee. UPJS MF will give the contents of clerkship to student. All the financial and legal terms are, in those cases, dealt between student and hospital/department.
9. Summer Clerkship forms and Evaluation of Summer Clerkship are available on the web http://www.upjs.sk/en/faculty-of-medicine/current-students/ and are to be submitted to the hospital/department of Clerkship.
10. In the first date of Clerkship necessary to report yourself at secretary of department/clinics/hospital where taking it.
