How to apply for the recognition of courses from your previous study?

Download Request for recognition of study subjects from previous study – GENERAL MEDICINE or DENTAL MEDICINE (GENERAL MEDICINE – transfer or DENTAL MEDICINE – transfer) according to your study programme
We recommend to use only computer, no mobile devices for filling the Request form
Open the file and fill in your personal information, fill in the name of your previous University, exact dates and academic years of study at your previous University
Tick corresponding boxes only with subjects you want to recognize
It is compulsory to fill in Corresponding Study subject name from your previous study with evaluation and exact date of passing only next to ticked boxes (containing subjects you want to recognize)
Continue with ticking subjects you want to recognize from your previous university with the Study subject name, evaluation and exact date of passing according to your Transcript from previous university
Check the Request form and Transcript from your previous university again
Save the file
Print out filled Request form
Sign printed Request form
