Application for recognition of study subjects from previous study

Manual how to fill in Request for Recognition of study subjects from previous study – GENERAL MEDICINE or DENTAL MEDICINE for transfer:

1. Download Request for recognition of study subjects from previous study – GENERAL MEDICINE or DENTAL MEDICINE according to your study programme
2. We recommend to use only computer, no mobile devices for filling the Request form
3. Open the file and fill in your personal information (fill in cells C4, C6, C8, D4, D6, D8), fill in the name of your previous University (fill in cell E4), exact dates (fill in cells F4, G4) and academic years of study at your previous University (fill in cell H4)
4. Tick corresponding boxes only with subjects you want to recognize (cell C15 – C130)
5. Is it compulsory to fill in corresponding Study subject name from your previous study (cell F15-F130) with evaluation (cell G15-G130) and exact date of passing (cell H15-H130) only next to ticked boxes (containing subjects you want to recognize)
6. Continue with ticking subjects you want to recognize from your previous University with the Study subject name, evaluation and exact date of passing according to your Transcript from previous University
7. Check the Request form and Transcript from your previous University again
8. Save the file
9. Send saved and filled request form by e-mail to e-mail address maria.spisakova1@upjs.sk
10. Print out filled Request form
11. Sign printed Request form
12. Submit Request with all attachments (hard copies) till 31st August 2021 at the latest to Ing. Mária Spišáková, The Study Department, Faculty of Medicine, Trieda SNP 1, 040 11 Košice, Slovakia
13. The Study Department will inform you within 30 days from the delivery of all documents mentioned in step 12 by e-mail to your e-mail address filled in request form
